Question for Loyola Education Systems Inc.

Our youngest brother, Josefino O. Cogal is the beneficiary of my late mother, Rosario Orongan O. Cogal of the plan she had. Josefino is now the plan holder of the PLAN TYPE ClASSIC GOLD. It was matured on July 4, 2016. How can we claim the plan benifit? Please provide us information what are the papers to comply for us to prepare. Thank you.
