Question for PET PLANS INCORPORATED

I'm a PET Plan holder bought in 1980's. Mine is "one time payment" that will mature after 17 years, sorry, I am not very sure if I remember it right. How can I claim the maturity value of my plan? Years back I received a Letter Notice from BPI informing me to claim my benefits at BPI Cainta Branch. Can I get the exact address of the BPI-Cainta Branch.
