Question for CONSOLIDATED PLANS INCORPORATED

I have a consolidated plan contract No. 302-P dated June 29, 1991. We had faithfully paid our dues and I believe your records would show that we have fully paid our contract price. May I be apprised of the status of this contract and what benefits are available to date. Thank you in advance for your response and explanations on this matter.
