Question for Manulife Philippines - Cebu Office

I've recently blocked-off my credit card where my Manulife payment is under ADA. I need to fill-up new form for ADA under new credit card, can I request please to send me the ADA form via email below and please advise if I can do the ADA transaction online or do I need to visit your Cebu office for this transaction.
