Question for AMA Computer Learning Center - Guagua, Pampanga

I'm undergraduate of Web Application Development year 2012. How can I get my copy of grades for the third semester cause the last time I'm trying to get my card, HR said they lost it and they want me to pay 500php to get another one or might as well just continue my study at ACLC. I'm planning to enroll next year and shift course at DHVTSU. How can I get copy of my grades? Thank you.
