Question for PET PLANS INCORPORATED

My Daughter Naomi (Scholar) is issued with your Company and we went at BPI Bank April 5, 2017 , the incharged said that I will submit Enhanced valued fund,statement account but I remembered that as payoo father I did not received those document. Please send the scholar fund for two semesters.Please advise us ASAP.
